A guide for families
Hospice Comparison Checklist: 6 Questions to Ask Before Choosing a Provider
Ask the right questions before choosing hospice. This free, printable two-page worksheet helps you compare three providers side by side and keep their answers together.
How to use the checklist
Bring the checklist to calls or visits. Write down each provider’s name, phone number, and the date, and use the same Provider A, B, and C labels on both pages. Ask each provider the same six questions and record specific answers.
Ask for examples of how help would work for your loved one, in the place where they will receive care.
Visits & hands-on help
How quickly can a nurse visit during an emergency?
Ask for the expected arrival time at your address, including nights and weekends.
How often will a nurse visit my loved one?
Ask about visit frequency, visit length, and changes when needs increase.
Are home health aides available, and how frequently?
Ask about personal care tasks, visit length, and coverage for missed visits.
Overnight help, training & supplies
What happens if we need help overnight?
Ask who answers, how to reach a nurse, and when an in-person visit is available.
What training will family caregivers receive?
Ask about medicines, comfort care, safe movement, and who to call with questions.
Who provides medications, equipment, and supplies?
Ask what is covered, who delivers and refills it, and about any family costs.
